Margin status Reason/Evidentiary Support: A positive surgical margin (PSM) significantly reduces the likelihood of progression-free survival, including PSA recurrence-free survival, local recurrence-free survival and development of metastases

چکیده

A positive surgical margin (PSM) significantly reduces the likelihood of progression-free survival, including PSA recurrence-free survival, local recurrence-free survival and development of metastases after radical prostatectomy in multivariate analysis. Moreover, positive margins are associated with a 2.6-fold increased risk of prostate cancer specific mortality. Careful inking of the outer surface of the radical prostatectomy specimen before macroscopic dissection (grossing) greatly facilitates the determination of margin status. A PSM can then be defined as cancer extending to the inked surface of the specimen, representing a site where the urologist has cut through cancer (See Fig. 5 below). PSMs are reported in between 10 – 48% of patients treated by radical prostatectomy for both organ confined and non-organ confined prostate cancer with the rates in the lower range typically found in more modern cohorts.

Positive surgical margins after radical prostatectomy: do they have an impact on biochemical or clinical progression?

OBJECTIVE To prospectively examine the effects of the margin status after radical prostatectomy (RP), the location, and the number of positive surgical margins (PSMs) on biochemical and clinical outcome, as even if there seems to be little debate that there is a higher risk of both local and distant recurrence of prostate cancer in the face of a PSM the significance of a PSM after RP is only fo...

Prognostic value of surgical margin status for biochemical recurrence following radical prostatectomy.

OBJECTIVE We evaluated the preoperative parameters to predict a positive surgical margin (SM) at radical prostatectomy for patients with prostate cancer. In addition, the prognostic factors for biochemical recurrence were determined in patients with positive SMs. METHODS We retrospectively analysed 238 patients with prostate cancer who underwent retropubic radical prostatectomy and bilateral ...
